What Is the Gastric Sleeve?

Gastric Sleeve Surgery (or Vertical Sleeve Gastrectomy) is a bariatric procedure in which 75-80% of the stomach is removed. It is now one of the most frequently performed elective bariatric procedures in the world.

Through this surgery, surgeons remove part of your stomach and stitch together the remaining portions to form a “sleeve” or banana-sized stomach. You will feel full much more quickly now that the sack is so small (about one-tenth the size of your original stomach). Losing weight will be easier since you won’t be able to eat as much.

Additionally, your stomach is removed as part of the surgery that makes a hormone that increases your appetite. The procedure isn’t very complicated. You can lose weight without exercising or dieting with a sleeve gastrectomy. Gastroplasty, or sleeve gastrectomy, is a restrictive type of bariatric surgery for the treatment of obese patients with a BMI greater than 40 or higher. This method encourages weight loss as it reduces stomach size, so it holds less food. Consequently, patients are able to eat less without suffering from digestive problems.

When patients with a BMI of 60 or more are extremely obese, a sleeve gastrectomy is sometimes applied to the first stage of a two-stage procedure. By doing so, the patient will lose a significant amount of weight; therefore, if the patient needs a second bariatric surgery, it is safer.

What Happens When You Have Gastric Surgery?

It is almost always done minimally-invasively with a laparoscope. An abdomen-long, thin tube is inserted into the body through several small incisions. A small camera and a light are attached to the tube along with various instruments. General anesthesia is used for gastric sleeve surgery. This medicine puts you into a deep sleep, requiring a ventilator during surgery in order to keep you breathing.

Your stomach will be divided into two unequal parts. In general, approximately 80 percent of your outer curved stomach is removed.

A staple or suture is then used to close the remaining 20 percent. This results in a stomach that is only approximately 25 percent as large as its original size.

This operation will take about an hour. Immediately following your surgery, you’ll be taken to the recovery room for postoperative care. You may usually feel a little confused immediately after regaining consciousness, but you are not in much pain, and your doctor will prescribe strong painkillers to help you have less pain after the operation. You’ll spend another hour or so in the recovery room. Nurses regularly monitor your blood pressure and postoperative tests. They help you get out of bed and take a short walk, as well as do breathing and movement exercises prescribed by your doctor.

Usually, small abdominal incisions heal quickly. Compared to a larger incision, the minimally invasive nature of the procedure allows you to recover more quickly.

It is possible to go home within two or three days after surgery unless there are any problems.

Taking a short time and having perfect results are the main advantages of this surgery.

Does this Surgery Work?

You can lose weight with gastric sleeve surgery in two ways:

Because of your smaller stomach, you feel full quicker and stop eating earlier. Therefore, fewer calories are consumed.

Due to surgery, ghrelin – a hormone associated with hunger – isn’t produced in your stomach, which makes you feel less hungry.

In most cases, you can expect to lose half of your excess fat within 18 to 24 months after having gastric sleeve surgery. Some people may lose between 60 and 70 percent of Trusted Sources.

Keep in mind that in order to achieve this, you will need to follow your surgeon’s diet and exercise plan consistently. As a general rule, your weight will remain ideal for a long time. You must strictly follow the instructions of your surgeon to ensure your surgery is as effective as possible.

Losing weight also has the benefit of reducing obesity-associated health conditions. Here are some examples:

Type 2 diabetes

  • Hyperlipidemia (high cholesterol)
  • Hypertension (high blood pressure)
  • Obstructive sleep apnea
  • A reduction in depression risk
  • Fatty liver disease

The following risks may occur after gastric surgery:

In general, the procedure of gastric sleeve surgery is considered to be relatively safe. In spite of this, in any major surgery, there is always a possibility of complications.

You may regain the weight you lost after gastric sleeve surgery if you:

  • Eat too much
  • Have an unhealthy diet
  • Not enough exercise

Additionally: Hemorrhage. A severe bleeding injury can lead to shock when it occurs inside your body or from your surgical wound.

Post-Surgical Tips That Are Essential

In the first days of your diet, you should eat only dilute drinks such as milk and liquids with very little or no sugar. You should consult your doctor following surgery about healthy eating and regular exercise suitable to your condition.

Frequently Asked Questions

A gastric sleeve is a permanent surgery and can last forever. The amount of weight you lose depends on the lifestyle after the surgery. It is even possible to lose 60% of the excessive weight within two years following the surgery.

Gastric sleeve is one of the types of bariatric surgery which helps people reduce their weight by removing 80% of the outer part of their stomach almost always by using a laparoscope. The remaining part is then sutured to form a banana shape stomach.

If you are a person between 17 to 70 years and a BMI over 40 (people with BMI over 35 with considerable health problems like diabetes type 2, high blood pressure, and sleep apnea or also qualified), which is considered morbid obesity, you can be a candidate for undergoing gastric sleeve surgery.

There are many pieces of research showing that this is the opposite and in fact, bariatric surgeries overwhelmingly decrease mortality risk especially in obese diabetic patients, though there are also studies indicating that for extremely obese people (with BMI over 62) these types of surgeries cut life-expectancy.

Since massive weight loss usually occurs following bariatric surgeries, the body looks thinner, so the face might look older than before.

Generally, the surgery is done under general anesthesia, so the patient does not feel pain during the surgery. You may feel moderate pain due to the cuts and incisions especially within the first week following the surgery which is usually manageable with pain killers prescribed by the surgeon.

Depending on life habits if a person regularly overeats after the surgery the stomach can stretch and grow larger, so the person can regain weight again.

Generally, after 1 month following the surgery, you will be able to eat normally. However, for attaining the best results after the gastric sleeve it is recommended to have a balanced diet and avoid eating greasy foods, foods containing sugar, alcohol, and spicy ones.

Although it varies from patient to patient due to the health and body condition and the diet guidelines which the person follows, In general, it takes 12 to 18 months to see a considerable amount of change. Usually, the majority of weight loss occurs in the first six months following the surgery and it can be as much as 50 percent of the extra weight.

There are many factors affecting the success rate of a gastric sleeve surgery like body health condition, surgery process and type, surgeon’s specialty and experience, diet guidelines and, the way patient follows them so, there is not a definite rate and varies from patient to patient but some piece of researches conducted indicate that weight regain could range from 5.7 percent at two years to 75.6 percent at six years.

It is recommended to manage your diet in a way to limit your daily calorie intake, especially for the first two months following the gastric sleeve surgery. Taking calories between 300 to 600 and not exceeding 1000 calories is advised so, try to avoid taking sugar, carbonated drinks, alcohol, caffeine, and fatty foods.

Gastric sleeve is a major surgery typically done under general anesthesia with the following drawbacks including  gallstone, hernia, internal bleeding, blood clots, perforation of stomach and intestines, vitamin or iron deficiency, leakage, infection, adverse reaction to anesthesia, lung or breathing issues, and hypoglycemia (low blood sugar).

Gastric sleeve surgery aims at profoundly changing the life of a person to a better and improved one, though it might have some risks along.

It is recommended not to drink caffeinated beverages for at least a month following the surgery as caffeine can cause dehydration and also plays a role in increasing appetite.

Although not all patients undergoing gastric sleeve experience hair loss, there is some advice to take for preventing hair loss following the surgery including:

  1. Increasing protein intake and consuming foods rich in protein
  2. Taking vitamins (especially A, C, and B9)
  3. Taking supplements or foods rich in Iron, Zinc, and Copper

Try to drink 6 to 8 cups of fluids every day to stay hydrated. Drink 64% of the fluid from soft liquid and the remaining 36% from full liquids containing protein supplements. Since you have got a smaller stomach keep a bottle with yourself and drink it slowly to avoid causing discomfort and pain.

Gastric sleeve has fewer complexities compared to other bariatric surgeries and is safest among other types. It rarely requires a second or revision surgery in case of operating under the supervision of a skilled and experienced surgeon. There are numerous certified clinics and hospitals operating gastric sleeves in Turkey. Since the Lira (national currency of Turkey) has depreciated due to the inflation caused by the economic recession, a possibility of undergoing surgery with the American and European standards and with lower prices has emerged, so thousands of people every year flow to Turkey for operating cosmetic surgeries.

For people who consider losing a significant amount of weight, a gastric sleeve is recommended though a gastric band is safer and has a less complex procedure.

While in the gastric band an adjustable type of band is wrapped around the upper part of the stomach to reduce the size, in gastric sleeve surgery, a large portion up to 80% of that is cut and removed.
